09-18-09 Cresskill, NJ - 3rd Alarm : September 18, 2009 - Just after 1000 hrs the Cresskill Fire Department was responding to a CO alarm on the east side of town and while in route Lt. James Pannone in command of Engine 5 notices a heavy smoke condition several blocks ahead of them. Directing their attention to find the source of the smoke they arrived in front of 113 Palisades Avenue with a working fire in a 1-1/2 split level.
 
Lt. Pannone notifies Cresskill Police of their findings and requested a 2nd alarm be transmitted which brings Tenafly Ladder 1 as the FAST and Tenafly Engine 1 to the scene. Lt. Pannone establishes Fire Command as his crew from E5 tags the hydrant next door to the house and stretches a 1-3/4 attack line as a search team is deployed. Tenafly Engine 1 established a secondary water source by hand stretching over 300 feet of 5-inch hose up hill. 
 
Dumont Engines 5,3, Rescue 3 and Squad 2 were the start of the 3rd alarm companies brought to the scene as fire conditions deterriated. The order to evacuate the structure was given by Lt. Pannone as heavy fire started to vent itself through the roof as the truck company was opening up. Additional companies that filled out the 3rd alarm were Bergenfield, Demarest, Haworth, Harrington Park, Oradell and Fort Lee Fire Departments. Fire was placed under control around 1130 hrs. No injuries were reported.

Working Fire Providence, RI 58 Hazard Ave September 14, 2009 : Providence, RI – Fire Alarm stilled out companies to the address of 58 Hazard Ave for a reported dwelling fire around 1315hrs on September 14, 2009. Engine 5 arrived to find this 30x50 2 story brick/wood dwelling heavily charged with smoke. Companies found the seat of the fire in the basement and began to stretch lines to extinguish the blaze. The fire was starting to extend upwards but was quickly doused by the aggressive attack. The OIC special called an extra engine and truck over the working fire. No injuries were reported and the cause of the fire is under investigation. The incident commander started to release companies 30 minutes after their arrival.

North Smithfield, RI Rollover with Entrapment Rte146 SB September 12, 2009 : North Smithfield, RI - A single car with 4 occupants was traveling southbound on Route 146 in the area of "West Acres" around 1430hrs on September 12, 2009. The vehicle somehow rolled over down an embankment coming to rest in a patch of trees. A witness reported that 1 occupant was ejected during the roll over, 1 was able to remove himself from the wreckage, and 2 occupants were still entrapped in the wreckage. North Smithfield Lieutenant Dave Mowry was on the first arriving apparatus and requested two more additional EMS rescues to the scene. Engine 1 arrived next due and went to work with their tools to open up the car to free the last occupants. Woonsocket Rescue 2, Lincoln Rescue 2, and North Smithfield Rescue 2 transported all of the occpants to level 1 trauma centers at Rhode Island Hospital and Hasbro Hospital.

August 2009 - MISC - Dog Rescue from Storm Drain : Somehow a dog found its way into a storm drain and somehow road construction workers found it. Truck 8 responded, lowered a ladder to a worker already in the drain and that worker used the ladder to grab the dog and bring it to safety.

3rd Alarm Dudley, MA 40 Fish Rd September 6, 2009 : Dudley, MA - Companies 2 and 3 were toned out around 0145hrs on September 6, 2009 for a reported fire at 40 Fish Road. The reporting party stated that he fled his home that was on fire and ran to a neighbors home to call 911. First due companies requested a second alarm to be struck on arrival due to the heavy overlapping fire from side 1 of the 1.5sty wood framed dwelling. The OIC soon after struck a third alarm bringing Company 1, Southbridge, Webster, Charlton, and nearby Quinebaug Fire from Connecticut. The firefighters stretched in several lines to try to darken down the blaze. The dwelling was located at the end of a narrow road which was a dead end. A tanker shuttle was established but was hampered by this narrow road. Tankers were shuttled in one by one as they were needed to refill the drop tanks. Firefighters were allowed to enter the dwelling to overhaul this severely damaged dwelling about an hour after they arrived. State Police Investigators will be assisting local officials in cause and origin.
